Our Patients

New patients are always welcome at Swan Medical Group. Please read below if you have not already visited us.

If a patient has a potentially infectious condition (e.g. influenza, TB, MRSA, chickenpox, measles ) please notify reception on arrival as isolation and surgical mask may be needed.

Please download our new patient form to fill out before you come in for your first visit.

Waiting Times

Patients who are particularly ill should notify staff on arrival, so that they can be made as comfortable as possible until seen by the doctor. A wheelchair is available in the surgery please contact a receptionist or nurse should you require one.


Authority scripts will not be sent via email. We will not prescribe restricted drugs via email. This service is only available if the request has been discussed as part of a consultation. Due to ‘doctor shopping’ issues, drugs of addiction will not be prescribed to new patients presenting to this surgery.

If you have not heard from us (time allocation), please contact the surgery to make sure the request has been actioned.

Repeat Prescriptions, Referrals & Results

It is in the best interest of your health to be seen by your Doctor whenever a repeat prescription is required. Patients should also return to see their Doctor for referrals and to discuss their test results.

We are a zero tolerance practice. We do not tolerate abuse of any kind. Patients who do not respect this policy will no longer be seen by our practice.
